Acute sinusitis (AS) in children is a common condition generally related to a viral infection of the upper respiratory tract.

The characteristic symptoms are runny nose, nasal congestion, cough during the day and at night, and in about half of the cases, there may be fever. These symptoms last less than 12 weeks (if they last more than 12 weeks, chronic sinusitis).

In most cases, this is a self-limiting process that only requires symptomatic treatment. Only in a small percentage of cases does a bacterial infection occur that requires antibiotics.

The diagnosis of AS in children is not straightforward as it shares symptoms with other common childhood nasal conditions, such as viral upper respiratory tract infections and allergic rhinitis. The physical examination is also not easy as some children may not tolerate nasal endoscopy.

Currently, it is estimated that the prevalence of chronic sinusitis (CS) in paediatric patients is around 4%.

Several factors are associated with CS, such as passive smoking and allergic rhinitis, although a clear and definitive causal relationship between allergic rhinitis and CS has not been established.

Sometimes, greatly enlarged adenoids, also known as vegetations, can act as a reservoir for pathogenic bacteria, favouring the appearance of bacterial sinusitis.

How is sinusitis treated in paediatric patients?

As mentioned above, medical treatment is the mainstay of paediatric sinusitis treatment.

Nasal hygiene, irrigations with saline solution, and paracetamol-type analgesics are recommended.

The use of antihistamines, nasal decongestants, and mucolytics is not recommended.

Antibiotics are only necessary in case of bacterial superinfection.
